Routine Care

This type of care is the most common one that a patient receives and usually the one that they start with. Routine Care is provided by the hospice staff at the place that the patient refers to as home. This can be a private residence, an assisted living home, etc. Under this type of care, the patient will receive regular visits from the members of our care team. These visits can range from one to seven times a week, depending on your or your loved one’s condition.

Continuous Care

This type of care is the highest that a patient can get in their home or residential facility. Continuous care is offered to the patient and their family during periods of crisis when their symptoms reach the point that they cannot be managed by routine care. During this time, a nurse will remain with the patient to help them relieve their pain and manage their current symptoms. The nurse can be there with the patient up to 24 hours a day.

Respite Care

Our respite care program is provided to patients to allow their family members and caregivers a short break from their current caregiving duties. During this type of care, the patients will be transferred from their homes to a skilled nursing facility that is contracted by us and will help manage the patient’s condition along with the hospice care they are receiving.

General Inpatient Care

Also referred to as GIP, General Inpatient Care is provided to patients that are experiencing symptoms that cannot be managed with safety in their homes or other home settings. This type of care is delivered in a skilled nursing facility, where nurses are fully available to manage their symptoms.
