Meet the members of our team that is going to take care of you and your loved ones.

To address the needs of our patients and their family members, we have assembled a multidisciplinary team of professionals that will work together to create a place where you are fully comfortable and independent. Our team is composed of physicians, nurses, home health aides, spiritual counselors, volunteers, and social workers.

This team will be the ones to manage the patient’s care and will do regular visits to the place the patient and their families call home. Together with the patient, their family, and their caregivers, they will work to develop a care plan that is patient-centered, addressing the different needs of the patient during the time they are under our care. Depending on these needs, the number of visits paid by our team and how many members of the team who’ll visit will vary.

Medical Director/Hospice Physician

The leaders of our hospice team and our physicians are in charge of developing a comprehensive care plan for the patient. They work in tandem with the patient’s primary care doctor to monitor their current health condition and vital signs. The physician is also in charge of prescribing the patients their medication that helps in relieving them of their pain and symptoms. They provide oversight of the patient’s plan of care and oversee the pain and symptom management of the patient, and coordinate with the attending physician when needed.

Nurses/ Nurse Case Manager

Nurses are the ones that spend the most time visiting the patients that are receiving care. As a direct result of this, our nurses work endlessly to establish a strong and trusting relationship with our patients and the members of their families. They make sure that every aspect of the patient’s hospice care is fully taken care of. Our nurses work with our physicians to make sure that they are up-to-date on your current condition and that the medication is properly managed. They provide hands-on physical care to ease symptoms and provide comfort for the patient, and coordinates care with the entire hospice care team.

Home Health Aides

The main role of our home health aides is to provide support in the form of personal care for the patient and their family members. They prioritize the patient’s overall hygiene and help with tasks such as feeding, bathing, grooming, shaving, and a lot more. Our home health aides understand how important privacy is to our patients and their loved ones. That is why they are very careful and sensitive when it comes to helping the patient during their baths and when they are in the toilet. They also assist and provide personal care such as bathing and grooming.

Social Workers

Our hospice social workers focus on providing our patients with both emotional and spiritual counseling. They help bridge emotional and social gaps and identify community resources for the family. They may also assist with end-of-life planning. They assist our patients with things such as explaining and educating facility placement, community resources, and more. They will also assist in easing the patient and their family with any kind of financial stress. Our social workers will remain free of any judgment and will listen to the patient’s feelings without them having to worry about being judged for what they think.

Spiritual Counselors

For our clients’ spiritual and emotional needs, our spiritual counselors are fully ready to support them. They will make sure that the patient’s spiritual needs are met and will act as the bridge between the patient and their religious community. They also offer bereavement support. With the diverse team of professionals that we have, we can provide a spiritual guide that is catered to the client’s religious beliefs.

Volunteers

We provide opportunities for the members of the community to give back by becoming a volunteer. They will assist our patients by providing them with companionship and assistance when the family needs to run errands or go to a brief appointment. They also do household chores, as needed. Our volunteers are trained to deliver and maintain personalized care by being receptive to the patient’s needs.

Additional Care

Care services may also include dietitian, nurse practitioner visits, or other therapies to meet needs specific to each patient.
